     for all children's choirs (must be notarized for Carol Choir, 
     Music Makers & Handbell/Handchime Choirs)
Youth Health form should be completed for youth 
     6th grade and above (must be notarized)

SonShine Singers - 3 forms to complete
Carol Choir - 5 forms to complete
Music Makers - 3 forms to complete
Handbells/Handchimes - 2 forms to complete
